Penis straightening and thickening: In the modern times, the people consider their external view and beauties in all senses and wish all sorts of operation that make them feel happier and more confident. The genital aesthetic operations, which are very popular in abroad for many years, increase in numbers in Turkey too. Birth, trauma or, a frequent occasion in our nation, post circumcision shape deformities are some of the most faced complaints.

Penis extension: The men who think that their penises are shorter than enough become afraid of entering a sexual intercourse under the anxiety that they may not grant joy to their spouses. As a result of this, performance anxiety, decrease in sexual pleasure, early ejaculation or erection problems may come to surface. In the most used method, some of the ligaments that bind the penis to the body are cut and, following, some the internally located penis tissue is taken out and covered with slid skin from sides. This method gives 2-4 cm lengthening to the non-erect penis. Sexual diet is required for about two months after the operation.

Penis thickening: This is one of the most frequent operations we perform. It is preferred due to easy and fast recovery. Lipid or skin-lipid layer is transferred under the penis skin for to obtain thickness. Taking and transferring actions are performed with 2 mm small incision leaving no traces. This method gives about 2 cm thickening to the non-erect penis. If the lipid is made from the base of the penis for this operation, then 2-3 cm lengthening is also obtained. The developed lipid injection techniques have increased the permanency rate of the injected lipid. Thus, no frequent repetitions are required. This operation may be performed under local anesthesia. A couple sexual diet weeks is required after the operation. Daily life however will start in 2-3 days. The readymade filling materials have also given successful results in the recent years. However the permanency and thickness is not as successful as in lipid injection.

Buried penis: The buried penis deformity does not require a short penis. The penis seems like buried due to the extreme accumulation of the lipid in base tissues; i.e. the most important reason is fatness. There come fast and net results without leaving any scar/trace thanks to the liposelection method, under local anesthesia, being applied to such surrounding base tissues. The secondary gain of this method is having a longer penis. Many of my patients have presented their astonishment due to such an easy solution of the problem. Another cause for such a situation is the badly performed circumcision. In case excessive skin is extracted during the circumcision, the penis may not go out. The penis skin should be extended for such patients. For to extend the skin, a skin graft placement or z-plasty to the skin in the base of the penis may be required. The skin graft is performed to complete the missing skin in the zone; z-plasty in turn provides an extension in the skin of that zone. The buried penis deformity rarely comes from birth. However in such cases, there are ligament tissues which force the penis to stay as buried. Lipid masses may be present in the crotches and the accumulated lipid tissue may even reach to the testis between the two legs. Since the body of the penis stays inwardly buried, the loose penis skin turns to a folded position and visible around the head of the penis. If the penis is buried in such a way but from the birth, the folded normal penis skin around the penis head may be mistakenly taken during the circumcision; thus, the body of the penis shortens in real terms and by no means taking the penis out may be possible.

Curved penis: The partially attached structures within the penis tissues will result as the curved penis from the birth and such penis may be distinct with a tendency to the attached zone. It comes out with the growing of the penis length especially in the teen ages. Depending on the degree of the curve, the sexual intercourse may become harder or the intercourse may not be possible. The diagnosis may be easily placed with the examination of erect penis. Birth coming penis curves may not be treated with medication. These patients require surgery for to correct such curve. The results are fully successful. The same kinds of complaints are frequently seen after the circumcision. It has the same kind of treatment. The skin of the scrotum hold the bottom of the penis in some men; even it may extend to the lower part of penis end. There may be a curtain, like the wings of a bat, between the penis and scrotum. This situation will make the penis seem even smaller. It is hard to use condom also. This curtain like connection may come from the birth or may form due to a badly performed circumcision. A tiny operation under local anesthesia corrects this situation properly.
